This Root Cause Analysis (RCA) investigates critical failures during the November 2023 HSM Plant hydraulic tube SAE flange relocation. Driven by severe schedule overruns, siloed leadership bottlenecks, poor shift handovers, and a lack of QA/QC hold points, the rushed execution resulted in widespread hydraulic leaks (missing hardware and inadequate torque) and crossed line routing errors. The included CAPA plan mandates distributed supervisory authority, formal handover reports, pre-cut tagging protocols, and strict QA/QC sign-offs to ensure future system integrity.
